My spouse and i entered vehicle and tried to put on seat belt when the driver's seat belt harness failed and became unbolted (spouse we driver). No force was used while trying to put on seat belt, simply used seat belt as normal. My spouse was able to re-attach the seat belt harness back into mechanism and ensured that the bolt was tightened.
